HEALTH CERTIFICATE issued by a duly licensed veterinarian or the U.S. Department of Agriculture dated not more than 30 days before arrival in the Philippines. It should certify that the animal is free from, and has not been recently exposed to, any dangerous or communicable disease, and that it has been given anti-rabies and other required inoculations.

CERTIFICATE OF AUTHENTICITY by the Philippine Consulate General. The Health Certificate and Import Permit should be presented to any Philippine Consulate General before shipment for authentication. The fee is $25.00. If sent by mail, payment should be in money order payable to the Philippine Consulate General. Please enclose a self-addressed stamped envelope for return mail.
